Q:  I lost the e-mail with the link to LHAT.  What is the link?

A:  The LHAT link is http://ehs.ucop.edu/lhat/.

Q:  I have tried to login to LHAT several times and each time I get an error message.  Why is this happening?

A:  It is possible that the web browser that you are using does not support the LHAT webpage.  We have found that Firefox and Chrome work very well in accessing the LHAT but if you are using Internet Explorer, only the more recent versions, IE 9 or better, support the program.

Q:  I am a lab worker and work for three different PIs.  Can I be a lab worker in more than one lab group?
A:  Yes, you can be a member of the roster for more than one lab group under a single PI’s hazard assessment or under different PI’s hazard assessments.  If the laboratories are in close proximity and share similar hazards, you will be issued one set of PPE.  If the laboratories are not close together and/or will required different PPE, you will be provided the appropriate PPE for each location.

Q:  I want to invite a lab member to join my lab group, but their name is not on the pick list for employees.  Can I still add them?

A:  Yes, if the person you wish to invite is not on the pick list, you may invite them using their e-mail address.  You need to click on the link just below the pick list (Blue letters – invite lab worker using e-mail).  This will open a second box to the left in which you may enter the e-mail address of the individual you wish to invite to join the lab group.  Once you have entered the e-mail address, click on “Invite Lab Worker.”  Their name should appear to the left on your invite list.

Q:  I did not receive an invite to join my lab group.  Is there a way for me to do this myself?

A:  Yes, you can initiate the process by logging into LHAT and searching for your PI’s last name.  You need to request to be associated with that PI.  The PI will receive an e-mail stating that you have requested to join their lab group and he/she must approve your request.

Q:  When I login to complete the training and print my voucher, the Dashboard shows that I am pending approval.  What does this mean?

A:  This means you have not been approved as a member of the roster for the lab group.  The PI or delegate, if there is one, will need to login to the LHAT and approve you.  This can be accomplished under the “Other Lab Actions” menu.  There will be “Approve” and “Disapprove” buttons under your name on the invite list.

Q:  I completed the hazard assessment, but it still shows it as a Draft.  What do I do now?

A:  Your hazard assessment will be listed as a “Draft” until you certify your hazard assessment.  The button to certify the assessment is at the very bottom of the assessment page.

Q:  I realize that I made an error in my hazard assessment but have already certified it.  When I try to change it, none of the buttons are active.  How can I amend my assessment?

A:  To amend a certified hazard assessment, you must scroll all the way to the bottom of the assessment and click the “Amend” button.  Once you have made the necessary changes, you will be required to recertify the assessment.  Your delegate may also amend your hazard assessment, but you will be required to recertify.

Q:  My PI completed the hazard assessment and I am on the roster for the lab group.  I tried to print a voucher, but the link is not active.  What should I do?

A:  The link for printing your voucher is not active because you have not completed all of the required steps.  Lab workers are required to complete three steps in the LHAT process.  Step 1 is to complete the required PPE training, which includes a short video and quiz.  Step 2 is the certification of the hazard assessment.  Lab workers are required to review the hazard assessment and then certify (at the bottom of the hazard assessment) that they have reviewed the assessment.  When Step 1 and Step 2 are complete, the print voucher link (Step 3) will become active and you can print your voucher.

Q:  I completed the hazard assessment and certified it, but I can’t print my voucher.  Why not?

A:  After you have completed and certified your hazard assessment, you must complete the required PI training, which includes a short video and quiz, before you can print your voucher.  The training (Step 1) can be launched from the Dashboard view.

Q:  My PI invited me to join his/her lab group and wants me to complete the hazard assessment for him/her.  How do I do this?

A:  Once you have been invited to join the lab group and have accepted the invitation, the PI must login a second time and assign you as the “Delegate.”  You may then complete the assessment.  The PI must login again to certify the hazard assessment.

Lab Manager / LHAT Delegate:

 

Q:  My PI instructed me to login to LHAT and set up his lab group.  How do I do that?

A:  You can’t.  The PI, or person of responsibility if the lab has no PI, must set up their profile and lab group in the LHAT.  Once they have set up the lab group, the PI can invite you to join the lab group.  After you have joined the lab group, the PI can assign you as the “Delegate” and you can complete the assessment and invite others to join the group.  However, the PI must certify the assessment.

Q:  I am the lab manager and already logged in and created the lab group.  I invited the PI to join the lab group, but now I am the PI and she is a lab worker.  Can we change roles?

A: Your roles can be changed, but it must be done by IT and involves the following steps. 

1. Login to LHAT and delete the lab group that you have created.

2. Send an e-mail to erm@ucop.edu, requesting a role change for both you and the PI. Your request should list your names and the role from and role to, such as John Smith, from PI to LW. 

3. While waiting for the role change, you should clear your web browser cache to ensure that your previous role is not retained. 

4. Once you have received confirmation that the roles have been changed, the PI needs to login, set up their profile and lab group, invite you in and then assign you as the delegate.

Principal Investigators:

 

Q:  I am a PI and have my own laboratory space.  In addition, I work in another PI’s laboratory.  Can I be a PI for my lab group and a Lab Worker in the other group?

A:  No, you can only have one role in the LHAT.  You can be either a PI or a lab worker, but not both.  If you need additional PPE in order to work in the other laboratory, due to differing activities or spatial considerations (different building), you should confer with the other PI and set up a second lab group within your own hazard assessment to capture the appropriate PPE.  You can have multiple lab groups within your LHAT and the PPE requirements for each can be different.

Q:  I am a PI, but was asked to join the roster of another lab group before I logged in to complete by hazard assessment.  When I try to login now, I cannot do so as a PI.  What should I do?

A:  When you were asked to join another lab group, your role in the LHAT became that of a Lab Worker.  To correct this issue, you need to request that the PI of the lab group remove you from their roster.  Once they have removed you, send an e-mail to erm@ucop.edu and request a role change from Lab Worker to PI.  While waiting for the role change, you should clear your web browser cache to ensure that your previous role is not retained.  Upon receipt of confirmation of the role change, you can login and complete the hazard assessment for your laboratory.

Q:  As the PI, I invited several lab members to join the lab group from the pick list and others using their e-mail addresses.  The ones that I invited using their e-mail address now appear on both the invite list and the roster.  How can this be fixed?

A:  When you invite a lab worker from the pick list, they are automatically recognized by the system and when they login they are moved from the invite list to the roster.  When you invite someone using their e-mail, once they have accepted the invitation and show up on your roster, you must go in and manually remove them from your invite list.  You can do this by clicking on the ‘X’ following their name.
