LHAT Assessment Re-Certification Tutorial

Principal Investigators (PI) are required to renew their Hazard Assessment annually.

The PI will receive a notification e-mail from LHAT one month prior the renewal deadline.

How do I re-certify my assessment? There are two options for re-certification:

I.   No Assessment Changes - Re-certification NO changes to hazard assessment

II.  Amend Assessment - Re-certification THERE ARE changes to hazard assessment

 

I. No Assessment Changes -

Re-certification – there are NO changes to the hazard assessment (Only available for a PI)

1) Select the No Assessment Changes button from home page

2) Select the Certify on behalf of The Regents of the University of California button

II. Amend Assessment -

Re-certification – THERE ARE changes to the hazard assessment (Available for both PI and Delegate)        [Ex. Working with new chemicals]

1) Select the Amend Assessment button from home page

2) Select Amend/Recertify button

3) Make the necessary edits to the Hazard Assessment form

4) Select Certify-I’m Done to complete (Only PI can certify. Delegate can save draft only.)

5) An e-mail will be sent to your lab regarding changes to the hazard assessment. They will have to review the changes.
