Controlled Substance Emergencies

The Principal Investigator is accountable for all controlled substances stored or used in their area. Theft or loss of a controlled substance is a criminal act that must be report immediately to:

Office of Environment, Health and Safety (EH&S): 415-476-1300

and

University of California, San Francisco Police Department (UCPD): 415-476-1414

If material is suspected to have been either stolen or lost, EH&S and UCPD will jointly investigate the incident.  Title 21 Code of Federal Regulations Section 1301.74 requires EH&S to report incidents to the DEA within one business day.  Failure to report may result in penalties.
