The Employee Hazard Report can be used by any employee who would like to submit a safety suggestion or report a hazardous workplace condition or unsafe work practice to Environment, Health and Safety (EH&S). Safety concerns should first be addressed with your supervisor. If your supervisor is unable to assist, contact EH&S.
This form can be submitted anonymously. To complete this form, fill in the requested information. EH&S will investigate all inquiries and determine whether mitigation is needed.
